To ensure a timely start into services please note the following:

• Complete the application in full (download application form here)
  
• Return the completed application to our office
  
• As soon as we receive the completed application we will contact you to set up an appointment for an intake meeting

The intake meeting (1 to 1½ hours) will be scheduled to:

• review your application confirm eligibility
  
• explain all of the services available to you through BIS

You will also be asked to complete release forms in order for us to access medical documentation regarding your acquired brain injury. If you have documentation of your own – please bring it with you to this meeting.
